Investigation Summary
At approximately 3:00 p.m. on October 30, 2002, Employee #1 was working with a crew on a construction job site, preparing the ground for a house to be built. He was driving a rented "Bobcat" tractor on a steep hillside, excavating holes to plant trees. As he maneuvered the tractor on the hillside, the vehicle began to slide sideways and then it rolled over, impaling him on a metal stake protruding from the ground. Employee #1 was hospitalized.
