Investigation Summary
At approximately 9:30 a.m. on October 5, 2005, Employee #1, a press operator, was operating the Rousselle Model Number SS-30-100 power operated press. At the time of the accident, Employee #1 was punching sheet metal with the press in the attempt to blank burner trays. At the time of the accident, Employee #1 was removing a burner tray from the press when the brake failed and the top ram and die came down, while the his left hand was in the point of operation. Employee #1 suffered an amputation of the middle finger on the left hand and was hospitalized.
