Investigation Summary
At approximately 9:10 p.m. on June 25, 2005, an employee was operating the electric stand-up forklift and walkie AR8, ATLET SELECT, Model Number IPO-T-200, Serial Number IPO 13495A in the Aisle Number 403 of Building number 4. When the employee was turning the forklift from westbound to the southbound, he lost control of the walkie and was spun around. The employee's foot was sticking out of the walkie and pinned between the edge of the walkie and the rack. The coworker responded to the incident and reported to the supervisor immediately, and emergency medical services were called. The employee was taken to Kern Medical Center and was hospitalized for 4 days and treated for crushed right foot.
