Investigation Summary
At approximately 12:00 p.m. on September 27, 2004, Employee #1 was feeding a 2 ft wide by 16 ft long by 1-in. thick piece of wood into a Delta Rockwell planer, Serial Number 215 TDR 1824 BCW F2, Model Number 96-849. The machine has automatic feed rollers in the front and back and was set up for 1-in. thick wood. The blade was covered with a metal guard, and was 15 inches away from the edge of the guard. In order to feed the wood into the machine, Employee #1 had to hold the center along the length of the work piece, 4 to 5 ft away. It took approximately 1 minute to finish one piece. Toward the end of a work piece, the wood would not go through the machine so Employee #1 pushed it harder and his glove was caught in the machine. Employee #1 was hospitalized with amputations to his first knuckle of four right fingers.
