Investigation Summary

Investigation Nr: 201119740
Event: 05/27/2005
Press Brake Machine Operator Lacerates Four Fingers

At approximately 11:15 a.m. on May 27, 2005, Employee #1, working for Paramount Farms, was fabricating a piece of 14 gage thick stainless steel metal, measuring 20 in. wide and 60 in. long, using a brake press machine (with identification information: #46-c- spec, Serial Number L-10541). While working on the metal sheet, Employee #1 lost grip on it and he became unbalanced. As he attempted to regain his posture, he inadvertently placed his right hand on the brake press point of operation and his foot simultaneously stepped on the brake press pedal. The press machine was activated and crushed Employee #1's right hand. He was transported to Kern County Medical Center and then moved to Sherman Oaks Burn Center. After one day, Employee #1 was transferred to Saint Joseph Hospital for further surgery on his right hand. He suffered lacerations to his four right hand fingers. Employee #1 was hospitalized for 18 days.
