Investigation Summary
On May 19, 2005, Employee #1 was working for Frye Construction, a framing subcontractor. He was on a four-foot-high rolling scaffold, taping drywall. With the scaffold's wheels not locked, he tried to roll the scaffold forward with his body motion, so that he could continue taping drywall in another area. Instead, he fell four feet off the scaffold to the ground and sustained a fracture of his left arm. He called a coworker, who was able to help him. Employee #1 was taken to Antelope Valley Hospital, where the fracture was diagnosed. He was admitted to the hospital and kept there for three days.
