Employee's Fingers Are Amputated in Press Machine

At approximately 12:15 p.m. on February 28, 2005, an employee was working on a Pacific Model Number 200-12, 200-ton press brake to do a 4-in. W by 12-in.-long piece of metal 900 bend on a 4"W x 12"L piece of metal sheet. While the employee was holding the piece of metal in the point of operation, he activated the press brake with a foot pedal. The employee stated that the piece of metal apparently slipped, causing him to place his hands in the point of operation when he activated the press brake. The employee's hands were caught in the point of operation causing injuries to both his hands. The employee yelled to a coworker who was approximately 15-ft away. The coworker went to the employee and saw that both his hands were caught in the die of the press brake. The coworker activated the machine to raise the die and remove the employee's hands from the machine. The employee suffered a partial amputation of his second and third fingers on both hands. After the accident occurred, the employee was taken to Sierra Vista Hospital, where a surgery was performed, but his fingers could not be reattached. The employee was hospitalized and treated for amputation.
