Employee Falls from Ladder and Fractures Elbow

On July 27, 2006, Employee #1 was ascending up an extension ladder. As the employee got to the top of the ladder, the ladder became misplaced and slid down the wall. The employee and the ladder fell 10 ft. Employee #1 sustained a fractured elbow and was hospitalized.
