Investigation Summary
On September 30, 2005, Employee #1, of Del Monte Foods Company, was using an elevated work platform to remove a plastic tarp from pallets of empty 1-gallon cans, which were being moved inside of the warehouse. Employee #1 exited the work platform and began walking on top of the pallets; however, he could not see that there was a gap between two of them (the tarp covered the opening). Employee #1 fell approximately 17 ft, which resulted in a fractured pelvis.
