Investigation Summary
On March 7, 2005, Employee #1, a mechanic, was working at the plate freezer of a fruit and vegetable processing plant. Employee #1 was making an adjustment to the line that was holding up a hinged bar, which was moving forward into the freezer section. As the hinged bar extended into the freezer, Employee #1's right middle finger tip was caught in the pinch point of the hinged bar and the freezers frame.
