Investigation Summary
On January 5, 2004, Employee #1 walked into a "t" aisle. He was looking down at his pants, and he did not see the forklift moving towards him. Another employee was driving a forklift in reverse. He was not looking in the direction of travel, and he did not see Employee #1 walking into the aisle, directly into the back of the traveling forklift. The forklift struck Employee #1, fracturing his legs. He was hospitalized.
