Painter Falls to Ground and Dies When Ladder Tips Over,

On April 13, 2004, a painter ascended a two-section extension ladder and was working near a roof eve that was approximately at an elevation of 10 ft 8 in. above the ground. The ladder was not secured to prevent movement, and an unknown event caused the ladder to fall, throwing the worker to the concrete surface. It was uncertain if the ladder moved or the painter's foot slipped. The worker sustained multiple injuries, including blunt force impact to the head, and died early the following morning.
