Investigation Summary
On April 25, 2003, Employee #1, a carpenter, was assigned to caulk the facia of the second story roof eave from the scaffold erected by the plaster company. Employee #1 fell to grade between the wall of the structure and the scaffold. It is estimated that the fall was 9 feet. There were no guardrails on the scaffold between the scaffold and the wall. Employee #1 was hospitalized with a fracture.
