Employee Run Over by Fork Lift

On December 11, 2002, Employee #1, a 27-year-old packer, was in route to drop off a box cap at the storage site in the next room to the packers' work station. As he entered the room, a fork lift was also entering the room in front of him. Employee #1 turned right once in the door and followed the wall until crossing the aisle to where the caps were stacked. Employee #1 was about to place the cap on the stack when his pant was caught and the fork lift traveling in reverse direction knocked him down and ran over his left ankle. The fork lift operator did not know that Employee #1 had entered the room and he did not see Employee #1 while making a sweeping right turn in reverse. The fork lift operator's hood blocked the view in making turns, and he did not use the provided mirrors on the fork lift. Employee #1 was hospitalized to treat his fractured ankle.
