Investigation Summary

Investigation Nr: 201095940
Event: 04/15/2002
Employee Hand Crushed

At approximately 11:15 a.m. on April 15, 2002, Employee #1 and several coworkers were moving a population stand to be packed up and shipped to another location. The population stand weighs 1630 lbs, and is a positioning fixture on wheels used to store manufactured parts. Employee #1 decided to work while his coworkers went to lunch. Employee #1 and his employer stated that it was not a normal procedure to work alone. While the stand was being rolled to another area of the plant, it got stuck. Employee placed a 4 in by 4 in board underneath the stand. Employee removed the wheels, the washer, and then put the wheels back in and raising the wheels so the bottom would clear the ground. Employee #1 was replacing the fourth wheel, when the 4 in by 4 in board shifted and the load landed on his left hand. Approximately 1 in. of Employee #1's left middle finger was amputated, and the index and ring fingers were crushed. Employee #1 was hospitalized and treated for his injuries.
