Investigation Summary

Investigation Nr: 201095007
Event: 10/05/2001
Employee Suffers Amptuation of Right Foot

At approximately 9:30 p.m. on October 5, 2001, Employee #1 was working at an establishment the manufactures building roof trusses. The trusses are processed through the finish truss roller at the completion of the manufacture process. The finish truss roller secures the truss joint connection plates. The trusses are then stacked by employees prior to distribution to customers. Employee #1 had been sweeping the yard area prior to the accident when a coworker asked him to help stack trusses. The two employees had stacked approximately 50 trusses and the coworker began aligning the stack. Employee #1 noticed trusses were stacking up in the feed area to the finish roller. Employee #1 entered the area where the trusses are feed into the finish roller to correct the trusses. Employee #1 did not shut off the finish truss roller and the roller was still operating while Employee #1 worked to correct the trusses. The trusses continued to travel toward the finish roller and Employee #1 feared he was about to be trapped between an oncoming truss and the machine. Employee #1 attempted to escape by climbing over the finish truss roller when his right foot was caught in and amputated by the machine. There were no witnesses to the accident. The supervisor was approximately 100 ft away from the finish truss roller and was not aware Employee #1 had entered the truss-feed area. Employee #1 was hospitalized.
