Employee's hand injured in punch press

At approximately 2:35 p.m. on October 14, 1996, Employee #1 was running a machine press called a back gear press. He bypassed the two-hand button operation by using a key, leaving his left hand free to push material into the press. While he was operating the press, it came down, lacerating and crushing his hand. Employee #1 was taken to the emergency room and was then transferred to Loma Linda Hospital, close to where he lived. The employer had given Employee #1 permission to shut off one of the hand controls, and admitted to the resulting hazard. The employer was issued a serious citation for failing to ensure that both hand controls on the press were in order.
