Employee injured when grinding wheel falls apart

At approximately 8:45 a.m. on September 27, 1996, Employee #1 was working with a grinding wheel when the wheel fell apart. He sustained a minor cut on the right side of his face, for which he was treated and released from the emergency room at St. John's Hospital. The employer was issued a general citation for failing to provide and ensure that face protection was worn when employees worked with grinding wheels.
