Investigation Summary
On August 24, 1996, Employee #1 was standing on top of a bench to replace light bulbs when the bench tipped over. He fell but sustained no injuries. However, as he went back to replacing light bulbs, metal fragments dropped from a socket into his eye. Employee #1 went to various treatment centers, and in each case, including an ophthalmology center, he was treated and released to go back to work. The employer was cited for failure to have an effective IIPP.
