Investigation Summary

Investigation Nr: 201082716
Event: 09/22/2000
Employees hand is caught in a conveyor belt and fractured

At approximately 9:15 a.m. on September 22, 2000, Employee #1 was coming back from his break and stopped at the apple conveyor. He was playing with the conveyor while talking to another worker. The belt conveyor is approximately 20 inches wide and was guarded at the sides. Underneath the conveyor belt is a take up roller which is where Employee #1 placed his hand. This take-up roller is not visible due to the guarding of the conveyor belt on the sides and due to the height of the conveyor. Employee #1 pulled his arm out from under the belt and roller, sustaining multiple fractures. He was hospitalized.
