Employee's fingertip amputated in butter packager

At 7:15 a.m. on August 7, 2000, Employee #1, a vacation relief operator with 17 years of experience for a butter manufacturing company, was assigned to work on butter printer #4, which put wrapped sticks of butter into cartons. The wrapped butter came down the line and was discarded by hand until it reached its required weight. Employee #1 continuously grabbed the butter off the line in groups of four wrapped sticks that were held by a small backing plate that moved the butter into the carton machine. Employee #1 was removing a packet of butter when the plate pushed his hand into an area where the plunger elevator met the frame of the machine. The plunger cycled up and down and caught Employee #1's left index finger, amputating it to below the nail. The employer was cited for a serious, accident-related violation of T8CCR 4002(a), for failing to guard that area of the machine.
