Employee Injured When Caught in Conveyor

Employee #1 was told to go into the canning area and clean the machines. The conveyor lines were to be cleaned between 6:00 p.m. and 6:30 p.m. when the employees working on the line went to dinner. The conveyor must be running to be cleaned and the employer had a lockout procedure. Employee #1's supervisor told him to clean the transfer conveyor. The bottom of the conveyor belt was 7-ft-8-in. above the floor. The top of the conveyor belt was 8-ft-6-in. above the floor. The belt drum was 10-in. in diameter and 16.5-in. wide. The supervisor placed a ladder near the conveyor and had Employee #1 clean the conveyor with a high-pressure hose with a 2.5-ft metal extension wand. Employee #1 began spraying the running conveyor with the wand when his left hand got caught in the conveyor, amputating it between the wrist and elbow. Employee #1 started to fall from the ladder and went to grab the conveyor when he realized his hand was missing. Employee #1 retrieved his own hand and went to the nurse's station. He was hospitalized for his injuries.
