Investigation Summary
Employee #1, a trainee, was riding on the steps of a Quinn Cat motor grader, serial #096406315, while a coworker who was training him sat in the cab and operated the machine. Employee #1 had mud on the bottom of his boots and when the grader began to vibrate, his foot slipped and he fell off the step. He was run over by the rear wheel, crushing his pelvis, rupturing his urethra, and lacerating his hip and buttocks.
