Employee fractures left wrist and back in fall

On March 28, 2002, an employer was installing racking/shelving units in a furniture warehouse. Employee #1 was standing on a unit installing shelving base plates. His coworker was operating a forklift in the area, and bumped one of the supporting legs of the shelving unit that Employee #1 was standing on. Employee #1 was not wearing his required fall protection at the time of this incident and was knocked off the shelving unit and fell 13 ft 6 in. to the concrete floor. He suffered fractures of his left wrist and fractures of his back at the Level-1, Level-2 and Level-3 areas. Employee #1 was hospitalized for three days. Causal factors in this accident appear to be the coworker not operating the forklift safely, and Employee #1 not wearing his required fall protection per OSHA and employer's rules.
