Employee Suffers Amputation Of Four Fingers

At approximately 11:15 a.m. on December 18, 2001, Employee #1was struck by a piece of material he was forming on an unguarded hydraulically powered "Sheridan Gray A-15" stretch wrap forming machine. The particular piece of machinery Employee #1 was operating forms the support ribs you would find in an airliner fuselage by stretching the material (also referred to as an extrusion) to a point where it reaches a semi-plastic state and then the extrusion is wrapped around a stationary die to form it. Apparently, the extrusion being formed struck the Employee #1's fingers when one end became dislodged from its tension cylinder. Once the extrusion snapped out of its tension cylinder, it snapped out with force sufficient to severe Employee #1's fingers between the stationary die and the extrusion, which had been under extreme tension. Employee #1 severed four fingers on his right hand in the accident. Employee #1 was hospitalized and underwent surgery in an attempt to reattach his severed fingers.
