Investigation Summary
On December 8, 2001, Employee #1 was painting a steel framed structure with a paint roller via a TELSTA aerial device that allowed him to paint the top structural supports, approximately 25 ft above a concrete floor. The basket of the aerial device was approximately 17 ft above ground level. As Employee #1 was painting, the latch on the door of the aerial device basket became unlatched. The door opened and he fell from the aerial basket to the concrete floor. The aerial basket did have a safety strap to secure the door, but was not utilized by Employee #1. Furthermore, Employee # 1 was not wearing any fall protection, which was required when using this type of equipment. Employee #1 sustained fatal head injuries in this accident.
