Empolyee injured after falling from stepladder

On October 10, 2001, an employee of Sign Tech was adding a new shelf to their metal stacking unit, located outside of the shop yard. The employee was using a 10 ft stepladder, in the opened and locked position. The employee was attempting to install a horizontal bracket that would support the shelf. He had one foot on a lower shelf about 8 ft high, and the other foot on the second rung to the top of the step ladder. The employee was using a prybar to pry open the shelf to install the bracket, when the shelf moved as he was pushing forward. He lost his balance and fell off the ladder and shelf unit. The employee fell about 8 ft on the concrete ground. As he fell he also struck his face/head on the bumper of a pick up truck that was parked next to him. The employee broke 2-3 ribs, fractured vertebrae, separated his right shoulder, and suffered trauma to his face and head. The employee was hospitalized for 5 days.
