Employee Finger Caught Between Wall and Forklift

At approximately 9:00 a.m. on October 13, 2003, Employee #1 was employed as a temporary clerk at the super market. The accident took place in the receiving area of the supermarket. Employee #1 had been employed for two days on the day of the accident collecting shopping carts and bagging produce. Employee #1's supervisor asked him and a coworker to unload water bottles off the pallets by hand. The pallet was sitting on the fork of the powered industrial truck. While the coworker went to look for something to cut the shrink-wrap, Employee #1 decided to operate the equipment to move the pallet. Employee #1's left thumb was crushed when it was caught between the control handle and the wall. Employee #1 sustained a laceration to his left thumb. Employee #1 was taken to the hospital, and treated for his injuries. Employee #1 was not authorized by the employer to operate the equipment.
