Employee Fractures Ankle While Driving Forklift

At approximately 9:00 p.m. on October 19, 2003, Employee #1, a grocery order selector, was hand loading cases of food onto pallets, and then delivering them to the docking area, using a standup forklift. Employee #1 received training on how to operate industrial trucks and he was authorized to operate the standup forklift. He was driving an empty standup forklift full speed eastbound along rack line D, toward the receiving dock, when he observed that there was another forklift traveling toward him on the same route with a load. In order to yield the way for the other forklift, he slowed down and attempted to cut through a cross way between rack line D and F, and continued heading toward the receiving dock. As he took his hand off the control and his foot off the brake pedal, he steered to the right, and the forklift continued to slide. He stuck his left foot outside of the operating platform. His foot hit the metal rack and became pinned under the brake pedal. He sustained a broken left ankle and was hospitalized for his injury.
