Machinery Crushes Employee's Fingers

At approximately 3:30 p.m. on August 8, 2003, Employee #1 was assisting the leveler machine operator in cutting sheet metal in various sizes at a customer's request. Employee #1 is responsible for loading the machine with rolls of sheet metal and placing wood to support sheet metal. Employee #1's left hand was pulled into in-running rollers on the Leveler machine and he suffered crushing injuries to his left third, fourth, and fifth finger resulting in the amputation of these fingers.
