Investigation Summary
At 8:15 a.m on September 9, 2002, Employee #1, the metal shop supervisor for a manufacturer of miniature model trains, was setting up a clip connector machine when the part ejecting mechanism became jammed and failed to retract. He was using an Allen wrench to push on the ejecting part when his right hand slipped and the cutting head came down, crushing his index finger between the top and bottom forming tools. Employee #1 went to a clinic for emergency care and was later admitted to the Western Hand Center, where the end of his index finger was surgically amputated. Upon further investigation, it became clear that Employee #1 did not de-energize the machine before attempting to clear the ejecting mechanism.
