Investigation Summary
Employee #1 worked in the production area in a steel processing plant. Employee #1's duties included banding finished slitter steel rolls and coils; operating the scrap winder machine; and packing the finished slitter steel rolls. At approximately 3:30 a.m. on September 14, 2001, Employee #1 was feeding scrapped steel wire through the level winder into the scrap winder. The level winder of the scrap winder was not guarded to prevent contacting the pinch point created between the moving level winder and the frame of the machine. Employee #1 pushed the jogging button and the level winder moved and crushed his left hand between the frame of the machine and the level winder. Employee #1 sustained three broken fingers. Employee #1 was not hospitalized but five days later required the amputation of his middle left finger.
