Investigation Summary
At 1:33 p.m. on October 11, 2002, an accident occurred at Paper Source Converting and Manufacturing in Vernon, California. The employer is engaging in converting large rolls of paper into napkins, tissues, and toilet paper. The process includes slitting, cutting and folding larges rolls of paper into the final product, packing them and shipping them out. The incident occurred at Napkin Line Number 2 in the production area. Employee #1 has been working for Paper Source Converting for 1.5 month at the time of the incident. Her job duties included packing the napkins and setting up the machine by feeding the paper through the rollers and cutters. On the day of the incident, the napkin machine has been jammed numerous times between the blower roller and the folding roller. Employee #1 reached inside the area of the rotating air blower roller to remove jammed napkins. Employee #1 did not deenergize the machine or lock it out before clearing the jammed napkins. The in-running blowing roller drew her left hand inside as she was reaching into that area to remove the jammed napkins. Employee #1 sustained an amputation of her left index, middle and pinky fingers, and was hospitalized.
