Investigation Summary
At approximately 12:15 p.m. on October 8, 2002, Employee #1, a machine operator in the extrusion department of Command Packaging, was re-webbing an extrusion and printing machine. The employer manufactures and prints plastic bags via an extrusion process and flexographic printing. Employee #1 started up the rollers to pass the knotted end of the webbing through to the other side where his supervisor was ready to take the knotted end of webbing. The employee was wearing knit gloves and stated that the glove and his hand got stuck in the rollers. His right middle finger was partially amputated at the distal phalanx. The ring finger on his right hand was fractured. The employee was taken to Technimed and then to the Western Hand Center where surgery was performed.
