Investigation Summary
At about 1:30 a.m. on May 25, 2002, Employee #1, a packing leadman with J & J Snack Foods, was adjusting a cookie-wrapping machine (CX Wrapper V FW 3410, Fuji/Formost) so the wrappers would cut at the correct location on the packages. A manager directed Employee #1 to loosen the tension on the chain from the back of the machine while the manager re-adjusted the registration on the computer. After loosening the chain, Employee #1 reached underneath to check the tension on the chain. The manager turned the machine on, causing Employee #1's right hand to become caught in the nip points of the chain and sprocket drive, and amputating his second and third fingers at the distal phalanx. The accident was caused by failure to follow lock/out, tag/out procedures during adjusting operations.
