Investigation Summary

Investigation Nr: 201056488
Event: 04/01/2002
Employee's finger injured while in conveyor belt

At 11:30 a.m. on April 1, 2002, Employee #1 was a Quality Control Technician for Overhill Farms, monitoring the production of frozen foods. Employee #1 was experienced at performing this job, having worked for one and one-half years. Employee #1 was monitoring the frozen food as it came out of a spiral freezer onto a conveyor belt, headed for the Packing Line area, and dislodging any food that become stuck. At one point in Employee #1's shift, an item became stuck near the sprockets on the side of the conveyor belt. For some reason, without deenergizing the conveyor belt, Employee #1 attempted to use her right index finger to dislodge the food item, but her finger became stuck in the sprockets of the conveyor. The Vernon Fire Department was dispatched to the factory and the victim was transported to a local hospital, where she spent four days and had two surgeries to correct damage done to the finger. Employee #1's right index finger was partially amputated.
