Employee Amputates Finger While Securing A Guard On Machine

At approximately 8:20 a.m. on August 13, 2001, Employee #1, a stacker and temporary employee from Headway Corporate Staffing Service, was assigned to work at Square H Brand Incorporated to replace a guard on a metal detector machine. As he was attempting to secure the guard, his left hand inadvertently contacted the moving chain, and his finger tips were partially amputated. He was stacking boxes of bacon product that was coming off the conveyor belt on the metal detector machine. Prior to the accident, Employee #1 had worked for the Headway Corporate Staffing Service for 3 years and had been assigned to work at Square H-Brand for 3 months. His work shift was from 4:00 a.m. until 12:30 p.m. His employer failed to check that the guard was securely in place. Employee #1 was taken to Stacey Medical Group, where he was treated.
