Employee crushes hand between forklift and a gate

At approximately 5:25 p.m. on November 2, 2000, Employee #1 was using a Barret Industrial stand-up forklift, Model RET-32-TTH, Type E, Serial Number A-22-75202 to move pallets at the loading area adjacent to the warehouse. He was backing the lift out of a chain-linked gated storage area. As he tried to maneuver the lift past the gate it started to swing closed. He took his foot off the dead man's pedal to stop the lift and at the same time reached his right hand out to push the gate open. His hand was crushed between the rail of the forklift and the vertical member of the metal gate. He sustained a fracture of the 1st, 2nd, and 3rd proximal phalanx, and a fracture of the 4th middle phalanx.
