Investigation Summary
At approximately 7:30 p.m. on July 14, 2000, Employee #1, a yard truck driver for Nisco Pacific, Inc., a warehousing and distribution facility, used a white yard goat truck to back and park a freight container at warehouse door #6. He went out through the back door of the yard truck and, while standing on the truck bed, disconnected the red air hose from the container's air brake release. He then dropped the air hose on the truck bed and started to driving away from the warehouse dock, not realizing that the red metal adapter of the air hose was hooked onto the chassis of the container. As Employee #1 drove away, he heard someone calling his name and turned his face slightly toward the left. As he did this, the red metal adapter flew inside of the yard truck through the open back door and struck the right side of his head. Employee #1 suffered a fractured skull and was hospitalized for nine days.
