Employee's foot crushed in leveler machine, later amputated

At approximately 5:00 p.m. on January 17, 1997, Employee #1 was feeding 3/16 in. by 31 in. by 31 in. carbon steel plates into a leveler with pyramidal rolls to straighten them. He was standing atop the feed table and pushing in the pieces when he slipped and his right foot became caught in the nip point of the roller. His foot was pulled into the machine and crushed. Employee #1 was transported to the hospital, where both his foot and the lower portion of his right leg were later amputated.
