Investigation Summary

Investigation Nr: 201034238
Event: 04/12/2002
Employee injured from falling crane

At approximately 9:40 a.m. on April 12, 2002, Employee #1 was guiding a truck mounted hydraulic crane carrying a large one-ton steel plate. The large steel plate was being inserted into various channels to divert the flow of sewage to other destinations within the treatment facility. The hydraulic crane was a Manitex, Model Number 1161 with a serial number of 11265 and a capacity of 22,800 pounds. As the crane lowered the load, a sinkhole developed under the outrigger of the crane causing the crane to tip over. The sinkhole had a dimension of 3-ft-by-2-ft-by-6-ft. When the crane tipped over, the boom fell causing the nylon lifting straps to drop. The nylon straps struck Employee #1, causing fractures in the femur bone of his left leg. He was hospitalized for three weeks after various surgeries to reconstruct the fractured bones in his leg.
