Employee crushed between metal rack and forklift

On July 5, 2001, Employee #1 was entrapped between the dashboard of a stand-up forklift and a metal rack at a plastics company warehouse. He attempted to operate a forklift, in which he was not trained since he could not find his supervisor to move it for him. As he got on this forklift, he inadvertently stepped on the brake pedal on the platform which caused the equipment to move. Employee #1 was hospitalized for his injuries.
