Employee's fingers amputated in chain drive

At approximately 1:45 p.m. on October 23, 2000, Employee #1 and a coworker, both maintenance mechanics at a canned fish food company, were repairing the plastic tabletop conveyor on a Busse unloader. The unloader was normally used to convey cooked cans from the cooking baskets to a can elevator, which then took them to labeling. During the repair process, operations workers attempted to start a nearby can unloader and elevator. When they did this, the chain drive for the plastic tabletop conveyor also actuated. Employee #1 had his right hand on the chain of this drive and three of his fingers were partly amputated. The conveyor had not been locked out before the attempted repair. Lockout training had been provided to both the maintenance and operations workers in 1998 or 1999, and a lockout lock had been given to each of them in 1999.
