Investigation Summary
Employee #1, a metal fabricator, was making a commercial sign for which he needed several pieces of angle iron. He used two finished commercial signs, each a 4 ft by 4 ft by 6 in. metal box, as a platform to gain access to the 10 ft high rack where the angle iron was stored. He was in front of the rack toward its south end when he fell 4 ft to the floor, fracturing his wrist and ankle. The cause of the accident was lack of a proper and safe means to access materials stored in high locations.
