Investigation Summary
At approximately 11:15 p.m. on June 26, 2008, Employee #1 was assisting a mechanic to repair a front-end loader. As he was working on the equipment, his left ring finger became pinched between a chisel and the pin hole of the front-end loader. Employee #1 was taken to Petaluma Valley Hospital, where he underwent surgery to repair a fractured bone in his left ring finger.
