Employee killed in fall from roof deck

On July 3, 1997, Employee #1 was nailing down a roof deck. He was wearing a safety belt attached to a lifeline when, for some reason, he unbuckled the safety belt. Employee #1 fell 24 ft onto a concrete slab and suffered a severe head injury. He was transported to the University of Maryland's shock trauma hospital, where he died on July 6, 1997. Employee #1 had received fall hazard training and been provided with the required personal fall arrest equipment.
