Employee killed after struck by a forklift

On February 6, 2002, Employee #1 of Luis A. Ayala Coon Sucrs, Inc. was directing forklift traffic on a four-way intersection during a marine cargo operation. He was standing in the intersection, giving directions to a coworker who was driving a forklift, when Employee #1 was struck by a forklift driven by another coworker and was killed. This coworker had poor visibility to the front, as his forklift was loaded with wood and raised forks. The coworker did not notice that Employee #1 was on his front.
