Employee killed in fall from elevated work platform

Employee #1 was on an elevated, job-made scaffold platform, filling in the last nail hole on a residential cathedral ceiling, when he fell approximately 20 ft and suffered serious head injuries. He was transported to the hospital, where he died the next day. There were no witnesses to the accident.
