Investigation Summary
Employee #1 and four coworkers were reroofing and shingling the 3-in-12 sloped roof of a 48 ft wide by 125 ft long sorority house. During the tear off, Employee #1 fell 22 ft from the 2-1/2 story building onto asphalt and was killed. The roof had multiple roofing staples that may have contributed to the fall. At the time of the accident, the wind was gusting to 20 mph. No form of fall protection was being used by any of the workers. The company corrected this by using a fall arrest system with roof anchors nailed into the trusses and lifelines and rope grabs attached to the anchorage.
