Employee killed in fall with extension ladder

Employee #1, a roofer, was part of a crew nailing shingles along the edge of a second-story roof. He was standing on a standard aluminum extension ladder when the ladder slid off the edge of the building to the ground. Employee #1 fell over 20 ft, striking his head as he landed. He was transported to the hospital, where he died of head injuries sustained in the accident. Employee #1 was not using any form of fall protection.
